Abstract:

Although there are several studies of the impact of employment of health professionals in large bureaucratic organizations, there has been significantly less research conducted that is both theoretically oriented and undertaken from an explicitly comparative perspective across professions, health care systems and cultures. In a previous paper we developed a conceptual model of the dynamic relationship between professions, organizations and clients based on an extensive review of the classic and contemporary literature on professions and organizations. In this paper we present comparative qualitative data gathered on the work experiences of health care professionals in Canadian, U.S. and French hospitals. We specifically elaborate on a typology of influence of clients on the orientation of health care providers to the community or to the bureaucracy.
