Abstract:

Objectives/Intent:
Patient Care II is the second of two courses that provides students with introductory direct patient care experiences. As a part of this process, students learn to identify and assess medical and medication-related problems. Students develop, implement, and follow-up on recommendations for identified problems in collaboration with a faculty preceptor.

Methods/Process:
Patient Care II incorporates both didactic and experiential learning during the Fall and Spring semesters of the third professional year. Under the supervision of a pharmacist coordinator, student teams work with older individuals from identified retirement communities to complete the core elements of medication therapy management (MTM). Students are expected to communicate written and/or verbal recommendations to the resident and their healthcare providers. For the didactic component of the course, students attend two small group workshops that focus on identifying and documenting problems as well as billing for services. Students use Google Docs to facilitate peer collaboration on documentation activities. Rubrics are used to assess various elements of the MTMS process.

Results/Outcomes:
Data obtained from rubrics will be used to evaluate student performance. Data gathered from billing activity will be used to describe the type of medication-related problems identified. Follow-up data will be gathered during MTM process to evaluate the percentage of recommendations accepted by the healthcare provider and patient.

Implications:
This course may serve as a model for introducing MTM principles into the experiential learning component of the PharmD curriculum.
