A New View of Privatization: State Efforts to Privatize Medicaid Services |

In this project, we examine the factors that influence how states utilize privatization as a means for delivering Medicaid services to its population. The states employ a variety of methods to operate and serve Medicaid populations, including the reliance on a wide array of private for-profit and not-for-profit organizations. Many scholars have studied the issue of state policy variation in Medicaid focusing on per capita spending as a key policy characteristic, but few have examined the role and reason why states choose one form of privatization over another. We utilize Ordinary Least Squares regression to examine the reasons for how and why states choose to contract with for-profit managed care organizations. We find that the structure of interests in a state explains quite a bit of state policy variation. In particular, we find that specific interest group strength is a key factor in explaining state policy choice variation as well as the composition of private health insurance markets in a state. Our findings engage the literatures on the role of interests and institutions in public policymaking with a particular focus on state policymaking. At the same time, this research has implications for the growing trend in state policymaking for choosing market-based mechanisms to deliver public services.. |
